We use information in three ways:
- Information FOR patients: Education and support to help patients manage all their conditions at home.
- Information FROM patients: Supporting remote care and gathering data through surveys and digital tools.
- Information ABOUT patients: Enabling decision-making by clinicians to support remote care, triage and population health management.
We are operating in 25 trusts and ICBs across the country, supporting over 70 conditions (in a single platform), nearly 300 symptom trackers (both hard and soft), are linked to multiple wearable devices, and are delivering in over 10 languages with more being added all the time.
